Sample Evaluation Forms

The University of Toronto campus ministry has put together an annual review template that can be adjusted to your local situation.  It includes a peer and student evaluation as well, which you could rotate around each year.  Think of it as a conversation tool that helps you keep in touch and name some strengths and growing edges.
